The Robert B. Green Campus Family Health Center, in collaboration with our promotores/community health workers, provide comprehensive healthcare to the most vulnerable individuals in our community. Often, our physicians recognize that the critical needs of their patients extend beyond what can be addressed in a typical office visit. Our community health workers, or promotores, gain an in-depth understanding of patients' circumstances—sometimes by visiting their homes or living environments—and work to identify and overcome barriers that hinder their health progress.
Armed with these insights, our team engages directly with patients and their families to determine how even the smallest financial support can make a significant difference. Our community health workers can then access the SHARE Fund to provide essential assistance on their health journey.
Even smallest financial support can profoundly impact the lives of San Antonio’s most vulnerable residents. For example, a grocery gift card can help a patient experiencing food insecurity obtain enough food to take their medication, or a hotel stay for a patient facing homelessness can enable them to prepare properly for a routine procedure. While these acts of support might seem small to some, they are invaluable to those in need.
